The application form must state explicitly that it is a medical form. The GP fill out the GP form. The applicant must attach the completed medical forms on the application form. The completed forms must be scanned and uploaded together with the application form.
A medical certificate must be obtained (signed by the applicant and a qualified medical officer, or a medical officer [physician] where the applicant attends a private medical clinic) from a qualified medical officer in a public health facility or hospital, and will be endorsed by the medical officer in the medical certificate.
Ascertain the applicant's ability to physically perform the TSC tasks by evaluating the applicant's physiological functional and measuring any physiological abnormalities. The GP will complete the relevant GP form (GP59 form) for the applicant, which is admitted in the applicant's PPS (personnel and pay summary) and shall be endorsed by the GP in the GP59 form.
All doctors are expected to adhere to the TSC guidelines on medical care of applicants. All doctors should be informed that a medical examination is compulsory/required during an applicant's enrollment process, unless they have a medical exemption that the GP in question has issued. The exam can be performed at any location, but should be performed in a public health facility or hospital.
